In my mind, Cain's memories of Apolo slowly deteriorate to a point where he no longer remembers his face, his hair, his eyes. He's just gone like that. But Cain never quite realizes this, he thinks he remembers him perfectly until he's shown a picture and that's when it clicks.
He effectively spent 11 years of his life looking for someone he doesnt remember the face of. The real Apolo could've stood in front of him and he would've been as valid to Cain as anyone else who vaguely resembles him. Promises to remember one another completly forgotten.
I also like to think that the real motivation behind Cain's obsession with finding Apolo stems from his desire to return to "normalcy", to easier and happier times. This becomes even more evident later on when Apolo returns but because he's not the same Cain doesn't feel fullfilled. He is convinced that fixing Apolo's memories will solve everything and is miffed when Apolo grows tired of the constant effort to change who he is.
