depends on how you want to play. i avoid the main quest(s) generally because theyre boring after the first time or two. cool to do for your first playthrough, but after that, meh.
for a more immersive experience, use mods. so many bugs and glitches take you out of the experience, but there are mods that fix those bugs. plus mods to make the game more pretty, make the citizens more interactive, add more substance to quests and/or add new quests and locations...
but if you just want the vanilla experience, the best advice i can give is to explore thoroughly (and save often. the game will crash at the most inopportune moments and i swear it does it on purpose). i mean, really take your time exploring every nook and cranny. you can find some neat stuff, throwbacks to the other elder scrolls games, lore, notes, weapons and armor that aren't connected to any quest... take the time to read the books in the game if you're really interested in the elder scrolls lore. there's some real interesting ones and some funny ones (the lusty argonian maid...)
start on a low difficulty if you're more interested in the exploration aspect, higher difficulty if you like the combat and want a challenge (plus higher difficulty = better loot in chests, generally).
lydia is a shitty follower. she gets in the way all the time. they all do, but she's particularly bad.
there is friendly fire. you can kill your followers, pets, and horses. unless they're marked as essential, you can kill them. so be careful about taking them into dungeons and stuff. they'll get in your way in tight spaces like caves all. the. time. and most have a relatively low level cap so a single sword to the head is enough to oneshot them after a point.
