They usually get banned fairly quickly, but if you are online at a time when they are making new accounts, then you may be unlucky (or lucky, I'm no judge) enough to see one or two.
I have, I saw some accounts with gory profile pictures (thankfully I think they've been banned), I'm glad you haven't seen any tho and I hope it stays that way!

Trust me, it happens. Usually it's once or twice a month and it comes in waves for about a day or two.
