Bathrooms in video games

I'm slightly confused as to the fact that in games (mostly open world games) bathrooms and whatnot aren't a thing. I get that it's a game and all but i feel like it would be good if at some point the game was just like "hey so you just ate 10 whole wheels of cheese, so, uh, nature calls. that along with your habit of just kinda eating anything you pick up, some of which was probably left there in the open for weeks, it sort of blew the horn for the proverbial floodgates to be opened, so you're gonna need to hit up the toilet... i would say real quick, but what with my previous point, well. you catch my drift, i'm sure. good luck, you'll need it" or something
