I have just over 900 hours in this game and these are my favourite items (subject to change):
(this is a general top 10, the numbers don't mean i think one item is necessarily better)
- Brimstone
- Soy Milk
- Mr. Dolly
- D Infinity
- Spindown Dice
- Godhead
- Sinus Infection
- Tractor Beam
- Binky
- Broken Modem
(i nabbed all the sprites from the wiki)
And these are my top 10 items, so not the best items in the game just ones I happen to favour. Some might be confused about Soy Milk being on the list, but it's a really fun item and can be very powerful with the right synergies, all you need is a good tear effect or two and decent luck and - BAM - you have boss melting build.

I have 1500h in tboi on my steam account :3
Soy milk was always one of my favorite items even before repentance and all the new synergies, it's just such a fun item.
admittedly, it took me a while to come around to it haha, but yeah now that i'm used to it it's sooooo good XD
